Elite Sports Permission Forms

The second class of Elite Sports for grade 9-12 will be starting on Thursday, Oct. 8th.  Any students enrolled in this class will require 2 completed and signed permission forms in order to fully participate in the class.  The first one is a School Cash online signed permission form in order to travel and participate in the Spartan Obstacle Course.  Please take a moment to visit School Cash to sign.   In addition the course venue requires a completed and signed waiver and liability form that Mr. Bowie will be distributing in the very first class, please ensure your child brings the completed waiver and liability form to school on Tuesday, October 13th.  It is mandatory that both forms be signed in order for students to participate.
